Bodypaint! One of the more amusing paint jobs I've seen was of a fried egg painted on a bald head.
Mad hats can be found in a wild array of styles. Ana Voog makes some of the more out-there hats that are posted on the crochet community -- I'm particularly fond of this one . There've been lots of others posted, but it's a sufficiently chatty place that it's hard to find things that aren't very recent.
